Every man has fears, be it flying, bugs or whatsoever, but what if you lose your manhood, that would be the worst.
But in this manga the protagonist doesn't lose his manhood, but his male face.
After an accident, Randoh was in a coma and had his face burned to not being recognized, a doctor wanted to restore his face(as a challenge) and gave him a girls face, which happen to be a girl he has crush on. But he awakens after a year, and notices the face and beats up the doctor, and beats him up some more because he wanted to remove his manhood.
But what he didn't know is that people believed he was dead, and that his parents moved away to recover from the shock.
He meats the girl he has a crush on and discoveries that she has a twin sister that run away from home, so he ends up being her sister, until the real one is found.
And the chaos begins, as he has to live as a girl, a big sister, and discovering how girls behave, despite being weak to women
The manga has quite fan service, also a girl with a penis that everyone would find discussing, but if you overlook that it is fun to read, I laughed a quite few time.
There is some romance but nothing worth parsing, the drama is average, if to say the least.
